Participants can also have more knowledge and meaningful conversations with doctors regarding creating the best possible options and outcomes.
Welvie My Life Letters SM is another program that enables participants to plan, document and then communicate their wishes regarding their future medical care in case these people are unable to speak for themselves. This Welvie My Life Letters SM program guides participants through the different types of medical decisions that can have to be made. Then, the program uses all participant’s responses to create fully written, personal SM letters that may be e-mailed, printed or even shared online for family, their doctors, or anyone else who just needs to know them.

Here you should do the following:
Step 1 – In the first field, you should enter your name. Make sure you enter your name as it is shown on your health plan ID card. Check the ID card if you are not sure.
Step 2 – In the second field, you should enter your middle initial name.
Step 3 – In the third field, you should enter your last name.
Step 4 – Now, in the fourth field, you need to enter your date of birth, in the month, day, and year order. You just need to click each option and select the right date.
Step 5 – Once you enter all the required information, click on the Continue button to proceed. Then you will need to follow on-screen instructions to complete the registration process.
Note: If you are attempting to access Welvie via your Health Plan or an Employer's Web site, the Employer or Plan can have a separate registration procedure in addition to the Welvie registration. In that case, Welvie recommends its users carefully read their Plan's or Employer's disclosure statement, which can include, between other things, a description of the business practices connecting to the collection as well as the use of a person's data.
