Step 2: Tap on the “Enroll Me” button

Step 3: To continue, you should have the following details to enrol, see below:

• Enter your First and Last Name

• Enter your Date of Birth (You must be 18 and older)

• You must provide your Medical Record Number (MRN) or enter the email address that is valid and listed in your medical record.

Step 4: If you used your MRN - Medical Record Number then once you successfully register for the portal, a “Patient Match Found” notification will pop up when the account is checked and verified. You should immediately check off the shown boxes to verify your identity and agree to the Terms of use of MyWellness service. Then, you must click on the “Next, Create Your Account” button to proceed.

Note that, if you use your email address instead of MRN, then upon your registration on patient match, the system will send you a 6-digit verification code to your email address that you used for verification. In that case, you should check your email inbox for the message that has included this code. Then go back to the MyWellness verification page or directly click the link given in the email. You should enter the code in the “*Verification Code” field.

Step 5: After that, an account set-up screen will appear on your screen. You should enter an email address right under the “I Need an Account” section that you have access to and then click on the “Sign Up” button.

Step 6: Now you need to fill out the extra details to finalize all the “Create an Account” fields. You will be asked to create a username, password, as well as some security questions on the same web page. Once you finish, tap on the “Create Account” button. Finally, you will automatically be redirected and logged into your MyWellness Patient Portal.
